Together, we are committed to breaking the cycle of homelessness and helping our clients achieve long-term self-sufficiency.Position Summary:LifeMoves | Branham Lane, located in south San Jose, supports single adults and adult families/multi-generational households. The program offers on-site services and connections to additional resources, including assistance with housing searches and meeting basic needs. LifeMoves aims to help participants achieve self-sufficiency through various support services, such as financial workshops, resume building, nutrition guidance, and conflict resolution, with the ultimate goal of securing permanent housing.Case Manager Duties:Develops, implements and revises individual case plans and consistently monitors progress toward client goalsProvide on-going supportive and/or case management functions in accordance with the case/treatment planAct as professional liaison for LifeMoves and our community partnersCommunicate appropriately as needed with probation officers, social workers and family membersParticipates actively as a team member in supporting clients and the facility as neededStay current and knowledgeable about external community resources, referrals and service linkagesProvides crisis management for clients; makes linkages for interventions as appropriateFacilitate at least one clinical or life skills group in addition to morning groupMaintains client and program records in accordance with applicable standards and regulations, grant requirements, etc.Maintains a high level of ethical conduct regarding confidentiality, dual-relationships, and professional statureParticipates in continuing education activities, remaining knowledgeable in area(s) of expertiseAttends weekly case consultation meetings and monthly trainings; meet regularly with supervisor to exchange pertinent information and receive supervisionOther duties as assigned by supervisorQualificationsBachelor’s degree in social work, psychology or related field, with a minimum oftwo years’ case management experiencePrefer minimum of 2 years’ experience in chemical dependency or other drug treatmentsExcellent communication and interpersonal skills; proven ability to collaborate with community partnersUnderstanding of the issues faced by homeless individuals and other marginalized populations in crisisAvailability to work occasional evenings and weekends.Experience working with mentally ill and dually diagnosed clients highly desirableAn understanding of ideas and practices that promote diversity, equity and inclusion; and a commitment to ongoing professional development in this area.Bi-lingual Spanish language capabilities preferredWe have a comprehensive benefits package, which includes Medical, Dental, Vision, Flex Spending Account, Dependent Care Reimbursement Account, Long-Term Disability, Life Insurance, 401K, etc.To learn more about our non-profit organization, check out our website at
