plays a crucial role in the LifeMoves organization by providing comprehensive support to clients transitioning from incarceration back into the community. Working closely with individuals from our emergency and transitional programs who have interacted with the criminal justice system, the

focuses on increasing housing opportunities and fostering successful reintegration by providing intensive case management. This involves conducting proactive outreach within the community to establish strong relationships with landlords, housing agencies, and other relevant stakeholders. Additionally, the role encompasses delivering housing search education through personalized counseling sessions, facilitating informative workshops, and mediating landlord-tenant interactions to ensure smooth transitions into stable housing. Operating both independently and collaboratively within a team environment, the Re-Entry Case Manager is dedicated to enhancing the agency's expertise in serving this population, ultimately striving to maximize housing options and tailor placements to individual needs. collaborates closely with the Employment Specialist and other external agencies to connect clients to a comprehensive range of resources aimed at promoting stable housing, economic self-sufficiency, and overall well-being.Key Responsibilities:Manage a diverse caseload of clients, including individuals with a history of criminal offenses, providing comprehensive support and guidance to facilitate their reintegration into society.Collaborate with legal and probationary agencies to coordinate services and monitor client progress, maintaining strict confidentiality and adherence to ethical guidelines.Conduct thorough assessments of families and individuals residing in emergency shelters, identifying their housing needs and preferences.Provide comprehensive, in-home case management services to participants enrolled in Housing First Programs, guiding them through the intake process, conducting regular assessments, and scheduling bi-weekly appointments to track progress and address barriers.Offer timely information and referrals to additional support services as necessary, intervening in crisis situations to ensure participant safety and well-being.Maintain accurate and up-to-date case files, documenting all interactions, assessments, and progress notes by agency standards and confidentiality protocols in Voyager.Collaborate closely with LifeMoves Case Managers and Program Directors to address programmatic needs, sharing insights and coordinating efforts to optimize client outcomes.Support the Program Manager in grant review processes, assisting in the evaluation of grant objectives and ensuring alignment with program priorities and outcomes.Work alongside the Housing Specialist to collect and analyze statistical data on clients served, contributing to the generation of comprehensive reports for internal review and external stakeholders.Conduct assessments with families and individuals in emergency shelters and assist them in sites within LifeMoves; as well as participating shelters that house our client population throughout the bay area, SMC, and SCC.Perform other duties as assigned by the supervisor, which may include participating in team meetings, conducting trainings, assisting with program development, representing the organization at community events, and more tasks as assigned to support the overall mission and objectives of the organization.Qualifications:• In-depth knowledge of the housing market trends specific to Santa Clara County and surrounding areas, including an understanding of fair housing practices, landlord-tenant rights, and the leasing process.• Demonstrated sensitivity to the unique needs and challenges faced by individuals re-entering the community after involvement with the criminal justice system.• Strong interpersonal skills with the ability to build rapport and establish partnerships with diverse stakeholders, including landlords, community organizations, and government agencies.• Proven ability to work both independently and collaboratively within a team environment, managing multiple priorities and deadlines effectively.• Excellent communication skills, both verbal and written, with experience facilitating workshops, delivering presentations, and preparing comprehensive reports.• Bachelor's degree in social work, psychology, counseling, or a related field preferred, with relevant experience in case management, housing services, or related fields considered an asset.• Commitment to the mission and values of LifeMoves, advocating for the rights and dignity of individuals experiencing homelessness and supporting efforts to create lasting solutions to homelessness in the community.We have a comprehensive benefits package, which includes Medical, Dental, Vision, Flex Spending Account, Dependent Care Reimbursement Account, Long-Term Disability, Life Insurance, 401K, etc.To learn more about our non-profit organization, check out our website at
