Hours:

40 hours per week with occasional evening and weekend shifts.

Work schedule option:

Monday - Friday, 12:00 pm to 8:00 pm OR

Monday - Thursday, 11:00 am to 8:00 pm (9 hours) and Friday, 8:30 am to 4:30 pm (8 hours), off every other Friday

Pay Rate:

$29.00 to $37.00 per hour

Education:

Bachelors degree in a mental health-related field such as Psychology, Social Work, or Counseling required. Masters degree in a mental health-related field or Doctorate degree (PhD/PsyD) in clinical psychology preferred.

As a Case Manager SBCM (Strengths-Base Case Management), you will:

Serve as the primary point of contact for all new referrals and enroll all new clients into the program.

Provide services to assigned clients within a Strength-Based Case Management model and ensure the program is following the fidelity of the model.

Work closely with various providers in the region or system to facilitate access to strength-based case management services for those identified as the target population to be served.

Oversee administrative and direct service tasks and duties, including outreach activities, monitoring program data, providing reports to Program Director, and other administrative duties as requested.

May conduct screenings, initial and ongoing assessments, crisis prevention and intervention, support with medication monitoring, individual/group counseling, and assist clients in accessing and using medical care, community resources when needed.

Provide crisis intervention as appropriate. During crisis situations, perform a risk assessment on each client (including non-open cases) and document accordingly on all clients evaluated. May initiate 5150s per CRF and County policies and procedures and make appropriate referrals to mental health services.

If assigned, on-call duties may include the need for services provided in the field, including evenings, weekends, and holidays.

Depending upon needs of the business and clients, may be expected to provide telehealth services.

Additional Qualifications and Skills:

Minimum of 2 years of experience working with a mental health population, especially with individuals with severe and persistent mental illness and co-occurring disorders is strongly preferred.

Ability to thrive in a fast-paced environment is essential.

Proficient in computer skills, including MS Office.

A valid California driver's license in good standing and eligibility to be an approved CRF driver is required to fulfill the essential functions of this position. In addition, proof of insurance is required prior to driving personal vehicles on company business.

Ability to pass criminal/background clearance.

Our Intensive Mobile Psychosocial Assertive Community Treatment (IMPACT, Downtown IMPACT, Mission Valley IMPACT, Senior IMPACT, and South Bay IMPACT) programs provide intensive, community-based case management services for homeless adults throughout San Diego County. We deliver fully-integrated services to clients diagnosed with a serious mental illness, as well as individuals with co-occurring mental health and substance disorders.

Our program utilizes the evidence-based practice of Assertive Community Treatment (ACT) to support our clients in the higher level of community care and to assist them in reaching their goals and living a life that is not dominated by mental illness. Our team is a model of care, providing treatment, rehabilitation, and case management supports.
