Job Summary : Provides leadership and supervision to RN Case Managers and staff at Tanana Chiefs Conference in primary care and specialty medical services. Establishes standards of practice for the provision of care management services for patients with both chronic and acute health care needs that require evaluation and interventions beyond routine primary care in order to achieve desired health outcomes. Ensures that case management services are conducted and documented according to standards that facilitate reimbursement for services according to CMS guidelines. Participates in TCC Clinical Services Division leadership activities by contributing to strategic planning and quality improvement initiatives. Position leverages population management tools and case management systems to provide excellent care management and a variety of enabling patient services. Provides system-level oversight of case management to ensure that patients with chronic or high-risk health and social needs have access to appropriate services. Adhere to the TCC Ch’eghwtsen’ model of service and guiding principle which requires providing timely and effective service along with the ability to interact with others in a way that inspires trust and demonstrates respect, compassion and empathy.

Representative Duties : Under the general supervision of the Director of Nursing, the job incumbent:

Provides leadership and direct supervision to TCC nursing staff in the delivery of case management and referral coordination services for chronic and acute high-risk patients.

Oversees program workload by collaborating with staff to ensure that referrals from medical providers are appropriate and timely and that criteria for the achievement of goals and discharge from case management are identified.

Reviews primary care team panels to identify high utilizers of services and, in conjunction with RN Case Managers and medical providers, designs methods to help these patients engage in preventative care with the goal of decreasing unnecessary ER or Urgent Care visits.

Provides direct patient education in person, over the phone, or via video-teleconference to support patients in managing their health. Creates standards for patient education and documentation for the case management program.

Facilitates regular meetings of the RN Case Manager group for case reviews and quality assurance and improvement projects.

Rounds often on RN Case Managers to make sure they are provided with the necessary support to be successful in the independent management of patients’ plans of care.

Provides formal orientation to case management and conducts annual performance evaluations of all direct reports.

Other Responsibilities : Performs other duties and responsibilities as assigned. Tanana Chiefs Conference expects that all nurses will maintain the ability to perform basic clinical patient care skills. Clinical skills competencies will be demonstrated annually per the requirements of individual roles and as needed in support of general patient care services.

Minimum Qualifications :

Must have a current Registered Nurse license for the State of Alaska;

Bachelor of Science Degree in nursing required, Master of Science in nursing is preferred.

Seven years of work experience as licensed RN.

Three (3) years of practical case management experience, preferably in a rural Alaska-based I.H.S. system.

Case management certification preferred.

Experience working in rural Alaska or providing health care to Alaska Native/American Indian populations is preferred.

Must be able to maintain strict confidentiality.

Must pass background check pursuant to federal Indian Child Protection and Family Violence Prevention Act requirement and State of Alaska DHSS background check.

Knowledge and Skills :

Proficiency using PC productivity applications (Microsoft Office applications) and electronic health record systems.

Knowledge of quality management and improvement processes for the evaluation and improvement of the quality of clinical services.

Strong verbal, written, and interpersonal skills.

Supervision : This position has supervisory responsibilities.

Physical demands : Job incumbent must be able to lift and carry up to 50 pounds in support of equipment and supplies. Position requires recurring bending, stooping, kneeling, reading, handling, speaking, hearing, and seeing. Position may require extended periods of standing. Incumbent may periodically lift patients, with or without assistance, in the process of providing medical assessment of patient conditions. Job incumbent must be willing and able to travel to remote locations throughout Alaska, and nationwide.

Summation : May be exposed to contagious diseases and risks while caring for emotionally disturbed and highly intoxicated patients. May be exposed to blood and other potentially infectious materials, which may result in possible exposure to blood borne pathogens.
