$65,000 - $85,000 (depending on experience)
Position Overview:
We are seeking a highly experienced and motivated Director of Case Management - RN to lead our case management department in Henderson, NC. This key leadership role is responsible for overseeing the coordination of patient care services, ensuring that patients receive the appropriate care in a timely and cost-effective manner. The Director will manage a team of case managers and social workers, driving quality outcomes, patient satisfaction, and efficient resource utilization.
Key Responsibilities:
Lead and manage the Case Management department,
including planning, organizing, and directing all case management
activities.
Develop and implement case management strategies,
policies, and procedures to ensure effective patient care coordination and
discharge planning.
Supervise and mentor a team of case managers and social
workers, providing guidance and support in their daily activities.
Collaborate with physicians, nurses, and other
healthcare professionals to ensure that patient care plans are executed
efficiently and effectively.
Monitor and evaluate patient outcomes, ensuring that
care delivery is aligned with best practices and regulatory requirements.
Oversee utilization management, ensuring that resources
are used appropriately and that care is delivered in the most
cost-effective manner.
Manage departmental budgets, including staffing,
equipment, and supplies, and ensure the efficient use of resources.
Participate in quality improvement initiatives and
hospital-wide committees to enhance patient care and operational
efficiency.
Ensure compliance with state, federal, and
accreditation standards related to case management and discharge planning.
Provide ongoing education and training opportunities
for staff to enhance their professional development and ensure high standards
of care.
Must-Haves:
Licensure:Current North Carolina or compact state Registered Nurse (RN) license
required upon hire.
Certifications:Basic Life Support (BLS) certification through the American Heart
Association (AHA) required upon hire.
Experience:A minimum of 3 years of nursing case management experience.
Nice-to-Haves:
Experience:Previous director/supervisory experience preferred.
Education:Masters degree preferred, or an equivalent combination of education and
work experience.
Qualifications:
Strong leadership and management skills, with the
ability to inspire and guide a team of case managers and social workers.
In-depth knowledge of case management practices,
discharge planning, and utilization management.
Excellent communication and interpersonal skills, with
the ability to collaborate effectively with multidisciplinary teams.
Strong analytical and problem-solving abilities, with a
focus on quality improvement and patient safety.
Experience in managing budgets and resources efficiently.
Commitment to ongoing professional development and
staying current with advances in case management and healthcare
regulations.
