Director of Home Health

Job Summary

The Home Health Director is responsible for the day to day business operations: supervision of care provided, maintaining administrative practices, assure compliance with applicable state and federal regulations and enhancing the growth and profitability of the agency. The ideal candidate will have previous home health and/or hospice management experience and knowledge of PDGM guidelines.

Reporting lines :

The Home Health Director will report directly to the Home Health Administrator/Regional Director of Operations.

Home Health Director Job Functions include, but are not limited to:

Organizes and directs the operations to assure availability and provision of care and services.

Directs and monitors quality improvement programs; continuously evaluates and improves processes, systems, and work flows. Evaluates systemic problems and develop action plans to address them. Reports results to superiors and subordinates, as appropriate.

Oversees, monitors and analyzes Agency’s outcomes; strives for improvement through the QI process.

Communicates with the clinical administrator/managers/supervisors/staff on a regular basis to assure that documentation and care is accurate and reflective of client condition and meets Medicare guidelines.

Directs and evaluate all operations and staff functions, i.e.; nursing, staffing, administration, etc.

Effectively working with others offering leadership by example and being a team player.

Operates and oversees the home health agency according to budget and within established fiscal parameters, including overseeing the productivity policy in place.

Ensures human resources functions including but not limited to interviewing, hiring, orientation, supervision, counseling, evaluations and staff development are performed according to company guidelines.

Performs patient care in patients homes, as needed.

Oversee Case Conference meetings including any follow up necessary.

Support with agency CHAP accreditation compliance as well as with Medicare licensing.

Participation in weekly Marketing and Business Development to increase admissions to the agency.

Minimum Qualifications:

Graduate of a Registered Nursing accredited school of nursing with a BSN degree is preferred.

Previous experience in hospice, home care setting and with two years management or supervisory experience. Proven ability to work within an interdisciplinary setting.

Registered nurse with current licensure to practice professional nursing in the state.

Complies with accepted professional standards and practice.

Has excellent observation, good nursing judgment and communication skills. Understands hospice philosophy and issues of death/dying.

Understands principles of pain/symptom management.

Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.

Complies and maintains current CPR certification.

Demonstrates excellent observation, verbal and written communication skills.

Preferred Qualifications:

Strong work ethic, positive attitude, professional poise, and a self-starter.

Substantial experience in managing complex and fast-moving projects with a proven history of reaching completion and achieving outcomes.

Understanding of clinical workflows and an ability to interact face to face with clinical staff to document & analyze practice-specific workflows.

Engaging presenter with excellent oral communication skills and presentation skills; must be comfortable presenting in front of both big groups and executives.

Demonstrated ability to communicate ideas clearly and concisely, interface and present to leadership effectively.

Promoting employee professionalism, ethical behavior, and personnel development.

Forward thinking in developing growth strategies, introducing new and innovative services, diversification of programming and automation of systems.

Excellent time management, organizational, and prioritization skills with the ability to balance multiple priorities. Strong analytical and problem-solving capabilities.

Maintain confidentiality of all PHI within guidelines of “need to know.”

Attention to detail and pride in delivering consistently accurate work.

Strong passion for patient care and healthcare, with empathy for patients and frontline staff.

Excellent Microsoft Office capability (Outlook, Word, Excel).
