Job Overview:
The Case Manager / Resident Guide is responsible for assisting residents with case management needs three days a week and resident guide responsibilities two days a week. The essential job functions of this position include:
Managing individual resident appointments and transportation.
Empowering residents to meet their needs independently.
Working closely with a multidisciplinary team.
Orienting residents to the program, including all necessary paperwork.
Assisting residents in setting and meeting weekly goals based on program phases and areas of growth.
Overseeing the operation of the home and integrating all operations.
Caring for resident needs between program hours with understanding and care.
Maintaining a calm and comfortable setting within the house.
