Job Details

We are seeking a dedicated and compassionate Family Medicine Physician/Primary Care Physician to join our growing team. This is a full-time, permanent position that offers an excellent opportunity to provide holistic, patient-centered care to a diverse patient population. This role involves providing comprehensive primary care services, including preventative care, acute care, and chronic disease management.

Responsibilities:

  • Provide comprehensive and continuous primary care to patients of all ages, including pediatric, adolescent, adult, and geriatric populations.
  • Diagnose and treat a wide range of health conditions and diseases.
  • Develop individualized care plans based on patient's health status, preferences, and goals.
  • Perform routine check-ups and health risk assessments.
  • Coordinate and manage patient care with other healthcare professionals as needed.
  • Maintain a strong focus on prevention and patient education.
  • Keep up-to-date with the latest developments in primary care and participate in ongoing professional development.
  • Adhere to all medical ethics and standards.
  • Document patient information, treatment plans, and interventions in patient medical records.
  • Participate in quality improvement, management, continuing education, and patient care programs.

Qualifications:

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited medical school.
  • Completion of a residency in Family Medicine.
  • Board certification or eligibility in Family Medicine.
  • Current, valid license to practice medicine in the state of Illinois
