Position Overview
A paraprofessional with knowledge and skills to effectively perform case management duties with individuals with substance abuse and who are evaluated to meet both financial and clinical criteria.
Job Duties:Maintain caseload of clients receiving substance abuse outpatient or intensive outpatient servicesComplete comprehensive, strengths, based assessment, and client needs assessment.Manages and coordinates services to meet the individual treatment needs of the client.Supports the clinical team in the development, implementation, revision, and the monitoring of the client's treatment plan.Facilities and coordinates services among other professionals and agencies to address client needs.Works with the treatment team to ensure continuity of care for the clients served by the program.Works directly with each client in the home to ensure that they receive the right care at the right time through the coordination of referrals, services, and the tracking of clinical outcomes.Advocates for the client when appropriate.Supports the client in obtaining appropriate entitlements and benefits such as SNAP, Medicaid, or medical coverage.Completes all requisite documentation in a timely manner.May be required to transport clients to various locations in a motor vehicle.Requires 24 billable units per weekMinimum Qualifications:
Bachelor's degree in human services field, with at least one year of experience serving people with substance use disorders or mental illness or people with other educational backgrounds who as providers, consumers, or advocates of addictions/mental health services have developed the necessary skills and knowledge to perform the tasks of Care Manager.These qualifications may be comprised of graduation from high school, GED or High Set combined with four years of relevant experience.A combination of education and experience may be considered.
