Clinical Intake
Coordinator
team to serve our patients and intake function across all markets for both day and night shifts. This position will be ONSITE at the facility location in Phoenix. The Intake Coordinator will work 3 days per week (36 hours) with the opportunity to work up to 40 hours. The NPH Clinical Assessment team has openings for qualified RNs for nights and days in all 3 marketsResponsibilities:Clinical Intake Coordinator (RN) ResponsibilitiesPerforms telephone preadmission for screening and clinical assessment.Performs insurance benefit verifications; assists with pre-certification as needed.Responds to inquiries and referrals regarding the facility and patient admissions within facility policy and set response timeframes and communicates provider recommendations to community partners, patients, and patients families and/or support systems.Educates clients, family members or significant others, referral sources, hospital personnel, external payers, and external case managers as to hospital benefits and services.Schedules and completes pre-admission, intake assessments, and all other intake paperwork.Communicates with other facility and company medical and psychiatric personnel to ensure appropriate placement recommendations and admissions.; negotiates approval for a clients stay through effective communication with hospital personnel and providersProvides accurate and ongoing assessment of patients status in the referral, admissions and intake, and utilization review processes.Performs ongoing assessments and physical function, emotional, social, spiritual, and financial needs on patients and implements crisis intervention and referral.Works with Business Development team to assess the needs of patients in accordance with the Pre- Screening Evaluation Form and initiates, implements and evaluates individualized patients.Builds a positive relationship with state wide behavioral health market including physicians, physician office staff and hospitals to market facility services provided by facility. Helps to educate the area discharge planners, physicians and other appropriate referral sources for admissions into the facilities; develops strong relationships with referral sources.Maintains a professional approach with confidentiality. Assures protection and privacy of health information as attained through written, electronic or oral disclosures. Complies with federal and hospital requirements in the areas of protected health information and patient privacy.Cooperates and maintains good rapport with nursing staff, medical staff, other hospital departments, and visitors as well as outside referral sources, hospitals and physicians.Seeks guidance and remains knowledgeable of, and complies with, all applicable federal and state laws, as well as hospital polices that apply to assigned duties.Complies with hospital expectations regarding ethical behavior and standards of conduct.Keeps abreast of all pertinent federal, state and hospital regulations, laws and policies as they presently exist and as they change or are modified.Understands and adheres to NPHs compliance standards as they appear in NPHs Corporate Compliance Policy, Code of Conduct and Conflict of Interest Policy.
Qualifications:Clinical Intake Coordinator (RN) RequirementsActive RN License or Masters Degree in Social Work, Counseling, or a closely related Social Services Field required, LPC or LCSW preferred.Minimum of six months in psychiatric and/or behavioral intake department and/or six months in psychiatric and/or behavioral Business Development and/or Marketing required; two years of psychiatric hospital department intake experience and/or Business Development experience preferred.Must demonstrate expertise in analysis, decision making, time management, oral and written communication skills, computer use, customer service skills, and strong ability to function efficiently and effectively in a high stress and fast paced environment.Familiarity with the resources available for use in assessing regulations and community resources.Good assessment skills and understanding of the insurance and managed care arenas.Ability to function independently and be a self-starter.Able to communicate the hospital objectives and goals to outside individuals and referral sources.Ability to provide and address social determinants of health (SDOH) issues to resolve problems encountered, including crisis resolution.Ability to assimilate information from a variety of sources, analyze information, and recommends courses of action to be taken.
