Rising Ground is headquartered in New York City.OverviewDepartment: Access and ReferralThe Intake Coordinator is primarily responsible for accepting and processing referrals for the Youth Reception Center (YRC), Juvenile Justice for Youth and Family (JYF), Supervised Independent Living Program youth (SILP) placements. The Intake Coordinator is an on-call position that screens intakes and decides independently based on eligibility and availability whether to accept a referral for further review by program staff.Reports to:
The Director of Access & Referral.Responsibilities:Duties:Intake TaskReceive, process, and distribute new intake referrals to appropriate programs (YRC, JYF and SILP)Responsible for collecting data and following up with intake referral sourcesDevelop strong relationships and partnerships with external referral sources (ACS)Maintain records of intake referral inquiries and admissions.Assist SILP admission teams with screening and scheduling pre-admission interviewsResponsible for processing new intake admissions to the appropriate programs (YRC, JYF and SILP)Responsible for entering new intake referrals through data management systems (CONNECTIONS, MYEVOLV and LEWIS)Responsible for assisting A/R team with managing on-call intake hotline and incoming resource referrals, includes non-business hoursQuality Improvement roleMaintain formal tracking system to measure agency service referrals performance as it relates to marketing and relationships with referral resources.Enters referral and agency intakes information in knowledge management systems (MyEvolv, CONNECTIONS, LEWIS, etc.)
Community Partnership
