We do this through 101 programs in 70 locations throughout New York City and Westchester. Our strong belief and commitment to diversity shows not just through our work with supported persons, but is integral to the building of a strong staff that reflects the communities we serve.The Outreach Intake Coordinator provides outreach services and support to fathers engaging them in the Fatherhood Program. Develop, maintain, and cultivate partnerships with community organizations, schools, and service providers to enroll 217 Fathers during the fiscal year. Furthermore, shall be responsible for all aspects of Fatherhood Intake process which includes but is not limited to engaging families before assigned to case worker and collecting/managing/organizing referral data.Responsibilities:Cultivate and maintain partnerships with culturally appropriate community organizations, schools and other service providers to assist with services referrals to participants.Conduct outreach to new referrals when referral source needs support in engaging/connecting a father to services. The Outreach / Intake worker will possess a basic level of understanding of the programs intentions, goals, and the AM / PM Fatherhood curriculum.Create, coordinate, and facilitate program outreach seminars, presentations, and workshops for members, potential members, and community partners.Lead marketing and community outreach special events (i.e., community events, job fairs) and represent the organization and program at such events.Engage clients utilizing engagement techniques to encourage, engage, and build connection with clients during intake process.Assess family/client availability and ensure each case is appropriately assigned to a case worker.Maintain an intake log listing all referred cases, referral dates, and assigned case manager.Effectively utilize PTS database to complete surveys and add demographic information to the system and first contact with the family, within 24 hours of the referral date, is entered as a progress note.Provide referral to fathers to various resources and services based on individualized assessments and assist in the coordination of services.Assist case workers to identify resources within the community for participating fathers.Provide follow-up assistance to fathers referred for services or resources.Complete administrative duties including maintaining hard copy files, auditing hard copy records, ordering food for groups, managing donations.The Outreach / Intake worker will attend all team meetings and serve on all committees as assigned.Essential FunctionsResponsible for recruiting and enrolling 217 participants per fiscal year.Provide orientation to new participants and complete required documentation.Responsible for maintaining a wait list (when necessary) of candidates when slots are unavailable. Associate degree;3 + years experience in accessing community resources and working with data systems, and will be familiar with the needs of the target population.Good verbal and written communication skills.Bilingual (English/Spanish) strongly preferred.Salary Range: $45,000 - $50,000Hybrid Schedule M-F, (2) remote (3) in-officeBenefits includes not limited (Medical, Vision, Dental.
