Responsibilities** This position is eligible for a $2,000 Hring Incentive**

The

Care Manager

will be responsible for providing comprehensive assessment and reassessment of youth, addressing their medical, behavioral, rehabilitation, social service, and long-term care needs. They will complete or revise the CANS NY assessment and develop person-centered Plan of Care. The Care Manager will also prepare crisis intervention plans, ensure the delivery of all aspects of care coordination, advocate for services and assist with scheduling medical appointments, and monitor attendance. They will facilitate comprehensive transitional care, involve family members in the planning process, refer youth and family members to appropriate community supports, and maintain timely and thorough documentation. Collaboration with Department of Social Service caseworkers, Catholic Charities program staff, and other community providers will be essential in providing quality care.

Starting Pay For This Position Is: $23.50/ Hour

QualificationsEducation and/or Experience Required at Entry:

Bachelor's of Science or Arts degree or NYS licensure and current registration as a Registered Nurse with two years relevant experience; or a Master's Degree with one year relevant experience required.

Demonstrated ability to work with people of diverse backgrounds required.

Degrees in Health or Human Services field preferred; experience working with an electronic health record system preferred.

Experience working with children and families preferred.

Thorough working knowledge of local and regional public and private resources needed.

An equivalent combination of education and experience will be considered.

Licenses/Certificate/Clearances:

Valid N.Y.S. Driver's License.

N.Y.S. Child Abuse Registry clearance.

Skills/Abilities:

Strong computer skills.

Creative problem-solving.

Negotiation skills.

Ability to communicate and interact with people from diverse backgrounds.

Working Conditions and Environment/Physical Demands:

Must possess mobility sufficient to drive to, and participate in, meetings at client's homes and/or community facilities which may not meet Federal accessibility standards for disabled individuals.

Manual dexterity sufficient to work with children/adults in a challenging and active environment.

Must have the ability to lift up to 10 lbs..
