Assertive Community Treatment Team, Enfield, CTABOUT THE PROGRAM:

Assertive Community Treatment (ACT) is an evidence-based practice that improves outcomes for people with serious mental illness who are most at-risk of psychiatric crisis and hospitalization and involvement in the criminal justice system. ACT uses a multidisciplinary team approach combined with assertive outreach in order to provide a wide range of services to clients, including teaching of life skills, vocational counseling, substance abuse counseling, medication management, therapy, assistance with accessing health care, and social and recreational activities.DUTIES & RESPONSIBILITIES:Conducts Clinical and Risk Assessments, LOCUS, Co-occurring Screens, and Functional Assessments.Collaborates with individuals to create individualized, person-centered treatment plans and documents clients' progress during treatment.Uses Motivational Interventions to assist clients in meeting their goals and moving along their paths to recovery, utilizing stage-wise interventions.Provides psycho-educational rehabilitation services group and individual for recovery from psychiatric and substance abuse disorders.Supports individuals with linking and engaging with natural (family, friends) or community supports.Develops crisis plans for clients and works with other staff to respond to crisis situations as needed.Provides skills teaching, such as assisting clients in obtaining/maintaining housing and utilities; learning how to use transportation services, planning and preparing nutritional meals, engaging and communicating with family and friends, being safe in the community, caring for children and other family members, and solving everyday problems that arise.Provides intensive skill-building interventions and support for self-management, self-advocacy, health and well-being, relapse prevention, and using community resources.Participates in family education and support activities.Maintains documentation in compliance with agency and program standards.Participates in staff meetings, supervision, training, and other meetings as assigned.Additional duties as required.QUALIFICATIONS:Education:

High School Diploma, Life experience.Experience:

2+ years working with identified populations. Must have lived experience of mental illness and be able to demonstrate significant Recovery in mental health or Substance Abuse. This role will require the worker to be willing to safely and appropriately share their lived experience in Recovery with clients.Licensure/Certification/Registration:

Valid driver’s license required and reliable transportation to transport clients in personal vehicle. CPR and First-Aid certification within 6 months of hire is beneficial.Training:

Basic computer skills required.COMPENSATION:
