Job Summary: Provide case management and direct instruction on community access skills, independent living skills, vocational skills and student specific skills. Help each student learn subject matter and skills that will contribute to his/her development as a mature, capable and responsible adult. Provide a positive, healthy and safe environment in which the student can achieve his/her maximum potential. Meet and instruct assigned classes in the locations and at the times designated. Plan a program of study that, as much as possible, meets the individual needs, interests and abilities of each student. Guide the learning process toward the achievement of curriculum goals. Create a classroom environment that is conducive to learning and appropriate to the maturity level and interest of the student. Encourage students to maintain standards of classroom behavior.
Essential Duties and Responsibilities:
Provide case management for students enrolled in the district's 18-21 Community Connections Program. Provide direct instruction in the community setting. IEP planning and progress monitoring for 18-21 student goals. Serve as a liaison among adult services' providers, students, families and HSD2 SPED staff. Collaborate with community partners, including DVR/SWAP. Connect High School students to community services. Conduct agency tours. State compliance activities including assisting and training secondary SPED staff on compliance with I-13 and I-14 requirements, conducting I-14 PSO interviews and audit, oversee I-13 audit. Attend various meetings including IEP meeting, interagency meetings, department/team meetings, PLC meetings. Attend CDE sponsored trainings. Motivate students to develop skills, attitudes and knowledge needed for college and career readiness. Communicate clearly in verbal and written formats with students, parents, and teammates. Develop lesson plans and instructional materials to meet the needs of students. Supervise students in out-of-classroom activities during the assigned workday. Must be able to lift and move up to 30 pounds. Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Possess or be eligible to receive Colorado Teaching License with appropriate endorsement. Candidate must be endorsed in the content area, passed the PLACE or Praxis test in the content area, and/or have 24 semester credits in the content area to meet Every Student Succeeds Act (ESSA) "highly qualified" status. Transcripts required. Teaching background with special education, at-risk, special populatios, differentiated instruction for diverse learners, scaffolding within classroom and community. Experience working with diversified populations (preferably youth), Job coaching experience. Criminal background check and District fingerprinting required. Knowledge, Skills, and Abilities
Good written and verbal communication skills Knowledge of Colorado workforce competencies Strong organizational and problem-solving skills Strong networking skills to collaborate with organizations, businesses, and agencies Financial/entrepreneurship/business management background helpful for school-based business development. Ability to become certified to transport students as needed Familiar with child labor laws
