Overview
Citizens Memorial Hospital (CMH) is recruiting a LPN to assist and triage patients over the phone when calling their primary care clinic.
Care Transitions answers and returns patient phone calls for CMH primary care clinics. The department is made up of two teams, schedulers and triage nurses, who work closely together.
A Typical day will include:
Answering and returning patient calls for triage and nursing advice. Relaying information and results between patients and their primary care provider. Assisting with facilitating medication refills, authorizations and questions. Identifying and connecting patients to organizational resources. Providing excellent customer service and behaving in a manner consistent with the mission of CMH. Performing other duties as requested.
The position is: Full-Time, Days
8:00 am-4:30pm Monday- Friday Job Requirements:
Education :
Associates Skills: Patient care experience License: LPN,LVN Benefits include:
