Virtus Healthcare Staffing is seeking a dedicated Nurse Practitioner/Primary Care Provider | US Applicants Only to join our partner medical center in Stockton, CA. This is an exciting opportunity to provide comprehensive care to a diverse senior patient population and contribute to a collaborative and innovative healthcare team.
Deliver Comprehensive Senior Care in a Collaborative Setting
As a Nurse Practitioner/Primary Care Provider, you will perform patient assessments, develop care plans, coordinate care, and contribute to quality improvement initiatives.
Responsibilities:
Patient Assessments: Perform comprehensive assessments, including initial, semi-annual, and unscheduled visits. Care Planning: Collaborate with physicians and the interdisciplinary team (IDT) to develop individualized care plans. Discharge Planning: Work with physicians and the IDT to facilitate smooth transitions from care settings. Nursing Home Care: Maintain and oversee care for participants in nursing homes. Care Coordination: Collaborate with providers and the IDT to ensure effective care coordination. Quality Improvement: Review data to identify areas for improvement and contribute to best practices. On-Call Coverage: Participate in a rotating on-call schedule. Qualifications:
Education: Graduation from an accredited School of Nursing. Certification: Certified as a Nurse Practitioner with Prescriptive Authority. Licensure: Active Nurse Practitioner State License. Experience: Minimum of two years of experience working with the senior population. Knowledge: Current knowledge of chronic care/geriatric issues and best practices. Other: Valid State Driver's License with a clean DMV record and proof of vehicle insurance. Benefits:
