• Complete MDS assessments accurately and timely
• Ensure compliance with federal and state regulations
• Collaborate with interdisciplinary teams for care planning
• Monitor resident progress and quality outcomes
• Provide guidance and support to nursing staff

• Registered Nurse (RN) license in NY (Required)
• MDS experience in a long-term care setting (Preferred)
• Strong understanding of Medicare/Medicaid regulations
• Excellent attention to detail and assessment skills
• A team player with a passion for resident care
