Patient Care & Service: Engage in essential patient care activities, collaborate with community health professionals, and assist with daily tasks such as personal care, meal preparation, light housekeeping, and errands. Communication: Maintain open and effective communication channels to report on patient status, treatment plans, and progress to both patients and healthcare teams. Documentation: Ensure accurate documentation of client care on activity sheets to uphold high standards of care. Compliance: Follow all required protocols and regulations set forth at the department and system level, including HIPAA and other local, state, and federal guidelines.
REQUIRED QUALIFICATIONS:
Valid New York State Driver's License. Home Health Aide certification from New York State Education Department.
PREFERRED QUALIFICATIONS:
Compassionate and patient-focused with a dependable work ethic. Strong critical thinking skills and the ability to make informed decisions with minimal supervision. Excellent communication and documentation skills. Ability to read, write, and speak English, and follow verbal and written instructions.
EDUCATION:
Completion of Home Health Aide certification as required by New York State.
PHYSICAL REQUIREMENTS: Medium Work - Exerting 20 to 50 pounds of force occasionally, 10 to 25 pounds frequently, and greater than negligible up to 10 pounds constantly to move objects; role requires frequent walking, standing, and squatting. PAY RANGE: $18.65 - $19.75 Location: Newark, NY 14513 The pay range is a good faith representation of current potential base pay for successful applicants.
