Salary: $120,000 - $129,000 annually
Cardiology Nurse Practitioner (1 position)
Salary: $134,000 annually

Work Schedule:
Monday – Friday, 8:30 am – 5:00 pm
On-call rotation every 4-6 weeks
Occasional weekend or holiday backup (rotation basis with extra mileage offered)
Role Overviews:
Palliative Nurse Practitioner:
Conduct home visits, with possible nursing home or hospital visits
Manage 4-5 patients daily, depending on travel requirements
Provide comprehensive symptom management
Engage in caring conversations with patients and families
Complete accurate and timely documentation
Opportunity to learn CPT coding (bonus skill)
Cardiology Nurse Practitioner:
Care for both inpatient and outpatient heart failure patients
Administer treatments such as IV Lasix
Focus on keeping patients out of the ER and comfortable at home
Prescribe and manage medications
Key Responsibilities for Both Roles:
Diagnose, treat, and manage pain and other end-of-life symptoms
Provide expert guidance on disease progression and treatment options
Prescribe and manage medications, including narcotics
Collaborate with an interdisciplinary team to deliver comprehensive care
Serve as an educational resource within the organization and community
Qualifications:
Current Nurse Practitioner license
Cardiac and/or heart failure experience preferred
Palliative care experience a plus
Minimum 1 year of related experience in hospice, palliative care, family medicine, internal medicine, geriatrics, oncology, or home health care

Pay Details: $120,000.00 to $134,000.00 per year
