Monday - Friday, 8:30 am - 5:00 pmOn-call rotation every 4-6 weeksOccasional weekend or holiday backup (rotation basis with extra mileage offered)Role Overviews:
Palliative Nurse Practitioner:
Conduct home visits, with possible nursing home or hospital visitsManage 4-5 patients daily, depending on travel requirementsProvide comprehensive symptom managementEngage in caring conversations with patients and familiesComplete accurate and timely documentationOpportunity to learn CPT coding (bonus skill)Cardiology Nurse Practitioner:
Care for both inpatient and outpatient heart failure patientsAdminister treatments such as IV LasixFocus on keeping patients out of the ER and comfortable at homePrescribe and manage medicationsKey Responsibilities for Both Roles:
Diagnose, treat, and manage pain and other end-of-life symptomsProvide expert guidance on disease progression and treatment optionsPrescribe and manage medications, including narcoticsCollaborate with an interdisciplinary team to deliver comprehensive careServe as an educational resource within the organization and communityQualifications:
Current Nurse Practitioner licenseCardiac and/or heart failure experience preferredPalliative care experience a plusMinimum 1 year of related experience in hospice, palliative care, family medicine, internal medicine, geriatrics, oncology, or home health careHow to Apply:

Pay Details:
$120,000.00 to $134,000.00 per year
Benefit offerings available for our associates include medical, dental, vision, life insurance, short-term disability, additional voluntary benefits, EAP program, commuter benefits and a 401K plan. Our benefit offerings provide employees the flexibility to choose the type of coverage that meets their individual needs. In addition, our associates may be eligible for paid leave including Paid Sick Leave or any other paid leave required by Federal, State, or local law, as well as Holiday pay where applicable.
