Description
I) Job Purpose
As Registered Nurse in our Healthcare Early Childhood program. You will support families and develop trusting relationships to help them meet their needs and build their capacity as new parents. In this role, you will impact families and their newborns to have a strong and healthy start, which in turn builds the strength of the communities we serve.
II) Essential Functions
A) Provide healthcare navigation and personalized care coordination to assigned partner hospitals or healthcare entities, including reviewing assessments (SDOH, Edinburgh, Substance Abuse, IPV), educating on pre/postnatal care and health topics, and collaborating on care plans between providers.
B) Facilitate resource referrals and loop closure by connecting patients to needed resources, following up on connections, sharing navigation plans with staff, and providing warm hand-offs post-discharge. Link patients with hospital and community resources such as Parent Pass, Perinatal Connections, support groups, and parenting classes.
C) Offer comprehensive support services, including emotional support, guidance on pregnancy symptoms and complications, access to hospital resources (lactation consultants, social workers, mental health services), and encouragement of health promotion and preventative measures.
D) Promote healthy behaviors to improve maternal and infant health outcomes, advocate for patient needs, ensure patient voices are heard, and document all interactions and follow-ups.
E) Attend rounding and multidisciplinary meetings to report on care plans, champion Perinatal Healthcare navigation, and facilitate team communication. Provide culturally sensitive care and support throughout all interactions.
F) Conduct recruitment and outreach activities to increase community awareness of perinatal connections supports and services. Assist with accessing eligible referrals from partner organizations as requested.
G) Participate in scheduled teaming and supervision meetings, as well as workgroups, committees, and coalitions as assigned.

III) Knowledge of Laws, Regulations, Policies/Procedures, Skills, and Abilities
A) Knowledge of child development, medical conditions, and diagnoses affecting children from prenatal stage to age 17.
B) Understanding of program eligibility requirements and services.
C) Proficiency in FERPA and HIPAA compliance, Texas laws on reporting abuse/neglect, and ethical nursing practices (Texas Nurse Practice Act, ANA Code of Ethics).
D) Excellent communication, analytical, and time-management skills, with ability to multitask and meet deadlines.
E) Expertise in protective factors, family-strengthening strategies, and trauma-informed approaches.
F) Proficiency in coaching, motivational interviewing, and case management principles.
G) Familiarity with community resources and ability to use relevant software systems (e.g., Microsoft Office, myAvatar, Salesforce).
H) Knowledge of Texas Health and Human Services Mental Health Rules, Medicaid Standards, DSM-5, and ICD-10.
I) Competence in Family Connects home visiting protocol and assessment of physical and psychosocial well-being domains.
J) Understanding of chronic mental health, integrated health, and optimal care trends for various populations.
K) Knowledge of medications, their uses, side effects, and ability to distinguish between medical and nursing interventions.
L) Ability to undergo regular competency assessments and chart reviews to ensure safe practice.
IV) Internal & External Customer Service
A) Interact and respond using a compassion mindset, awareness of need for felt safety, and consideration of all variables affecting families and teammates.
B) Work effectively with families/adults from various cultural, linguistic, socio-economic, cognitive, social-emotional, and educational statuses.
C) Build and maintain effective working relationships with internal and external customers, including families, characterized by mutual support, open communication, trust, and respect. Always considering what is best for families in decisions about treatment and support.
D) Discuss uncomfortable and potentially conflict-producing, stressful topics in a professional and effective manner with internal and external customers and maintain courteous interaction.
E) Must demonstrate, compassion, emotional control, and safety practices including suicide safe care.
V) Travel
A) Adhere to MHMR Mileage and Travel reimbursement policy and any other aspect regarding travel.
B) This position requires daily travel including home, school, and community-based visits for families as assigned.

B) Defined Education: Nursing
C) Preferences: Bachelor's degree in nursing; MSN, MPH, or other advanced degree; certification as lactation educator or consultant; Spanish-speaking

G) License/Certifications: Current Texas Registered Nurse license

VIII) Agency Requirements
A) All staff are required to participate in agency Emergency Preparedness and Environmental Safety programs and may be assigned by their department as a key/essential staff level function during critical events or for the purpose of sustaining business continuity.
B) This position may require temporary or permanent re-assignment to any MHMR Tarrant facility as determined by program needs and/or the Division/Director.
C) Assigned work hours may change as the needs of the agency change.
D) The Functional Title of this position may change as the needs of the agency change.
E) All work will be completed within the scheduled work hours. All non-exempt (hourly) employees are expected to clock in and clock out for each work shift, no work should be done off the clock.
F) MHMR reserves the right to change, add to or eliminate positions as it deems appropriate.
G) Employment is at will, as well as agency needs may change.
H) Agency dress code is to be followed at all times.
