Salary based upon experience and degree
Responsibilities:
Assist individuals in obtaining and maintaining integrated community employment under the HCBS waiver.
Assist individuals sustain paid employment in an integrated setting in the general workforce that meets personal and career goals.
Engage in person-centered employment planning to help develop vocational goals.
Document the delivery of SEMP services, review documents and prepare Staff Action Plans.
Attend Life Plan meetings and communicate with other OPWDD staff regarding progress and outcomes for individuals receiving SEMP services.
Develop a positive working relationship with individuals involved with the program.
Have and/or gain knowledge of barriers to employment faced by individuals with disabilities.
Discuss how to accommodate for those barriers, and how to advise/educate an employer regarding those barriers
Promote a culture of inclusion and belonging.
Requirements:
Bachelor’s Degree in a relevant field and related experience working with individuals with disability is preferred. An Associate’s Degree in a relevant field and related work experience will be considered. Must have reliable transportation. Valid NYS driver’s license is required as driving is a part of this position. Solid customer services skills or have relevant counseling training/experience. Must be able to effectively organize, prioritize and use your time wisely.
