* SUMMARY *:
At NNHOPES, our mission is to provide affordable, high quality medical, behavioral health, and support service to all. The Title X-Community Care Navigator supports this mission by working alongside the program supervisor to provide a combination of administrative and patient care support to patients that qualify for Title X services.
* MINIMUM/PREFERRED REQUIREMENTS *:
Due to real and potential conflicts of interest that exist when HOPES Behavioral Health providers render services to their coworkers, it is HOPES policy (Policy HR0005) that HOPES employees may not access HOPES Behavioral Health Services. For the same reason, current Behavioral Health patients/clients cannot be employed by HOPES. Accordingly, you hereby acknowledge and understand that if you are receiving services from a HOPES Behavioral Health provider at the time you are offered and accept employment with HOPES, you have the option of rescinding your acceptance of employment or finding a new behavioral health provider outside of HOPES before starting employment with HOPES.
Minimum of one (1) year of work in healthcare setting with a focus on patient care or program reporting; or equivalent combination of education and experience. Prior healthcare experience such as medical assisting, nursing, community health work, front desk, or other medical experience a plus. De-escalation skills and/or experience in de-escalating patients Ability to obtain CHW 1 certification within 6 months of hire required. Valid BLS required prior to hire, per HRSA compliance. Able to communicate effectively in oral and written form. Previous experience presenting and providing training a plus. Experience utilizing most Microsoft computer programs, particularly Excel and PowerPoint. Prior experience with grant management and quality improvement reporting a plus. Proficient in Spanish/English, written and verbal preferred but not required.

JOB RESPONSIBILITIES/DUTIES:
Title X Family Planning Programs aim to improve access to family planning and related preventative health services. The Community Care Navigator will engage with medical providers and patients to monitor Title X funded services, and engage in the retention and outreach of Family Planning and Preventative Healthcare qualifying patients. This position leans heavily toward administrative duties such as data tracking, reporting, and training while using these duties as opportunities to engage patients in healthcare services.
