A social services agency in New York City is looking to add a new Licensed Practical Nurse to their growing Adolescent Program. Working under the Nurse Manager, the Licensed Practical Nurse (LPN) will be responsible for overseeing medical care management of the youth in residential and family foster care.

Responsibilities:

The Licensed Practical Nurse (LPN) will:Identify health needs of adolescents in care and their caretakers through home visits and other direct means of interactionEstablish for the adolescents in care a medical home that will address current and future health care needsAct as a liaison between multiple providers in the provision of health services for our adolescents in careEstablish a compassionate environment by providing emotional, psychological, and spiritual support to clients and familiesResolve client problems and needs by utilizing multidisciplinary team strategiesDocument client care services by charting in patient and department records; on-going auditing of medical records to ensure compliance with regulatory agenciesCommunicate with clients and multidisciplinary team by providing client related health informationAdminister medication and monitor the youth's response to prescribed medicationPerform other duties as assigned.Qualifications:

Graduate of an accredited Practical Nursing programCurrent American Heart and/or American Red Cross CPR certificationSolid assessment, clinical, and documentation skillsPatient orientedGreat interpersonal skillsExcellent communication skills (written and verbal)Strong attention to detailHighly organizedDesired Skills:

Bilingual (English and Spanish)
