A social wellness agency in New York City is actively seeking a new Licensed Practical Nurse (LPN) for a great opportunity with their growing Assertive Community Treatment (ACT) team. In this role, the Licensed Practical Nurse (LPN) will be responsible for providing management and administration of medications for persons with severe and persistent mental illness.Responsibilities:
The Licensed Practical Nurse (LPN) will:Work with clients to develop an understanding of their symptoms and medicationsMonitor for side effectsParticipate in hospital discharge planning meetings and assist in the follow up of medical treatment post hospitalization or ER visitsAcquire discharge paperwork from hospitals and provide copies to team leader, prescribers, and program assistantsMaintain compliance with incident reporting guidelinesReport all incidents in a timely manorProvide education to clients and staff regarding standards of medical careProvide linkages to appropriate medical care providers when applicablePerform other duties, as neededQualifications:
Graduate of an accredited Practical Nursing programNYS Licensed Practical Nurse (LPN)Proficiency in Nursing and Medication AdministrationKnowledge of Treatment and Rehabilitation methods for persons with severe and persistent mental illnessSolid assessment, clinical, and documentation skillsPatient orientedGreat interpersonal skillsExcellent communication skills (written and verbal)Strong attention to detailHighly organizedDesired Skills:
1+ year of experience in a Treatment and/or Rehabilitation setting
