Become a part of our caring community and help us put health firstThe National Director, Clinical Pharmacy is a strategic pharmacy leader responsible for developing, implementing, and improving clinical pharmacy programs for the Primary Care Organization (PCO) that transform outcomes for seniors in medication-related care, integrate pharmacy care with primary care, and effectively support our rapidly growing clinical pharmacy workforce.This clinical leader will play a critical role in ensuring optimal patient care, high-quality health outcomes, and improved cost and utilization in pharmacy-related care, serving as a national clinical program leader within our growing Complex Care function under the PCO's Chief Medical Officer. This role reports to the National Director, High Risk Patient Management.Responsibilities will include:Lead comprehensive clinical pharmacy service development, execution, and operations in support of primary care patients and care teams served by the Primary Care Organization, including building pharmacy team-based care integration & programs across medication reconciliation, disease management, medication adherence, polypharmacy, and other crucial areas.Evaluate current programs and drive roadmap and execution progress for continuous improvement, improving patient outcomes and achieving business goals.Collaborate with national clinical & operational leadership, shared services, and collaborating business units such as CenterWell Pharmacy to ensure integrated pharmacy programs are well-designed, well-executed, and deliver impact on business and clinical goals.Act as the national pharmacy subject matter expert (SME) and clinical liaison to the national clinical leadership, practice operations, training, process, and analytics to optimize our pharmacy approach.Co-Chair the PCO's Pharmacy & Therapeutics Committee, guiding pharmacy-related clinical policy development and recommendations.Lead continuous improvement and feedback loops to identify best practices and drive improvement opportunities for current programs.Serve as an internal and external champion for the program areas under purview.Support pharmacy performance improvement activities and align pharmacy services with improvement quality metrics and Medicare part D performance.Collaborate with stakeholders to improve pharmacy-directed HEDIS measures (Med Adherence) and programs to improve medication optimization and promote age-friendly care.Maintain current knowledge of pharmacy/medical trends within ambulatory/primary care and Medicare.Required Qualifications:Doctor of Pharmacy (PharmD) with active Pharmacist license with the Board of Pharmacy in the appropriate state and willingness and ability to be licensed in additional states.5-7+ years of progressive clinical pharmacy program leadership experience as a manager, director, or higher overseeing clinical pharmacy services and programs.Proven ability to drive comprehensive pharmacy program effectiveness and oversight across multiple sites and geographies, with concrete impact on clinical and business outcomes.Experienced team leader with success leading large, collaborative efforts and teams, including direct-reports and matrixed functions.Preferred Qualifications:Additional post-graduate training and certification beyond PharmD, such as MBA, MHA.Reside in proximity to a Conviva/CenterWell market location(s).Experience working in a value-based care, senior care, and/or Medicare managed care.Workstyle:This is a remote position with travel to Conviva/CenterWell clinic locations across the US as needed.Scheduled Weekly Hours:40Pay Range:The compensation range below reflects a good faith estimate of starting base pay for full-time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job-related skills, knowledge, experience, education, certifications, etc. $164,700 - $226,600 per year.
