, established in 2009, is a growing, behavioral healthcare treatment platform treating adults with substance use disorder and co-occurring conditions under the Northpoint Recovery brand and adolescents with mental healthcare conditions under the Imagine by Northpoint brand. The Company serves patients with commercial health insurance and is an in-network model. Northpoint has grown from two facilities to fifteen today entirely on a de novo basis with plans to expand rapidly in both existing and new markets. As a key member of the Northpoint team, the Counselor will oversee the therapeutic services provided to a caseload of patients. This individual will act as the treatment plan coordinator for the multi-disciplinary team in the development and implementation of the master treatment plan and the discharge plan for each patient. The Counselor will provide individual, group and family therapy for assigned patients, as required by contractual and licensure obligations. This person will actively communicate with support staff, treatment teams, and outside agencies on treatment and safety issues pertaining to patients and their family members. The Counselor will complete all documentation associated with these duties in a professional and timely manner.ESSENTIAL RESPONSIBILITIES AND DUTIES INCLUDE BUT ARE NOT LIMITED TO THE FOLLOWING:Provide counseling, assessments and group facilitation to individuals and familiesCoordinate all aspects of direct clinical care from admission to dischargeConsult with the multidisciplinary treatment team to provide quality patient careProvide orientation and education to patients, families and others to aid in the therapeutic processCollaborate with assigned patients in the development of written treatment plans; include patient and family involvement in assessment, treatment, education and discharge planningMonitor patients on an ongoing basis to continually assess needs and facilitate treatment and interventionsDocument clinical assessment, diagnosis, treatment plan, diagnostic summaries, and progress charting in a timely manner to assure comprehensive and quality patient careActively participate as a member of the multidisciplinary treatment team and maintain effective working relationships with employees and peersMaintain professional boundaries with patients at all timesOversee and participate in the quality assurance and compliance activities of the facilityProvide clinical updates to utilization review and representatives of managed care to best coordinate reimbursement of treatment servicesCoordinate with community based providers and referral sources to ensure optimal levels of care and treatment outcomes, post dischargeCase staffing and individual case supervisionFacilitate individual and group therapyAdhere to all company policies and proceduresMaintain confidentiality in accordance with established policies and procedures and standards of careOther clinically appropriate services and special projects as assignedQUALIFICATIONS/REQUIREMENTS FOR POSITION:Master’s Degree of Mental Health, Social Sciences or commensurate experience requiredMust hold an active substance abuse or mental health licensure in the state where treatment services are renderedMust maintain applicable state licensure requirements at hire, and for the entire duration of employmentMinimum of 1 (one) year experience in relevant fieldAbility to work with a diverse population and possess strong interpersonal skills and knowledge required for treating patients assignedMust hold strong personal boundaries and able to build rapport with patiensMust understand and adhere to the ethical standards of the respective licensure governing boardPREFERRED KNOWLEDGE AND SKILLS:Excellent organizational abilitiesExcellent written and oral communication skillsAttention to detail and accuracyPatient service oriented (both internal and external)Creative and persistent problem solverAble to handle confidential material in a reliable mannerAbility to interact and communicate with individuals at all levels of organizationStrong interpersonal skills to handle sensitive situations and confidential informationAbility to multi-task and prioritize workload in a fast-paced environmentProficiency with Microsoft Office Suite and EMR systemsREQUIRED CERTIFICATES/LICENSES/REGISTRATIONS:Must hold an active substance abuse or mental health licensure in the state where treatment services are renderedMust maintain applicable state licensure requirements at hire, and for the entire duration of employmentMust be at least twenty-one (21) years of ageMust maintain current nationally accredited CPR/First Aid/AED certificationMust have a valid driver’s license and safe driving recordSUPERVISORY RESPONSIBILITIES:NoneJob Type:
