Central Ohio Primary Care is seeking a highly motivated Registered Nurse (RN) committed to patient center care with strong customer service skills to fill a Triage Specialist position in our busy Care Navigation Center in Westerville, OH. The Triage Specialist conducts phone triage services for the patients of COPC offices that coordinate with COPC Care Navigation. Conducts a thorough telephonic nursing triage for acute and chronic medical complaints. Utilizes electronic health record (EHR) EPIC for documentation of patient interactions regarding triage protocols, appointment scheduling, homecare nursing instruction, and collaboration with other healthcare team members. This is a full-time role working 9:30a-8p or 12:30p-11p, four days/week with rotating weekends. 5 days/week is available. Hybrid option once training is completed.Essential Functions and Responsibilities:Conduct telephone triage for patient health complaints using an electronic triage assessment tool (Schmitt-Thompson)Navigate multiple electronic platforms to assess current health complaint, patients' electronic chart, PCP appointment scheduling, and ancillary center scheduling have appropriate and timely documentationDocument phone call details in tracking system for department data tracking and collection.Assess, identify, and close patient care gaps.Follow and execute all policies, procedures, and job responsibilities instructed by direct supervisor.Develop positive and effective working relationship with COPC staff, offices, and physicians.Qualifications:Education, Licensures & CertificationsActive and unrestricted registered nurse license in the state of Ohio.Two years minimum Register Nurse experience in a hospital and/or medical practice settingKnowledge, Skills & AbilitiesExperience in telephonic nursing triage, preferred but not required.Exceptional active listening, communication, and documentation skills, required.Excellent customer service skills, and mind-set, required.Ability to collaborate with a team, required.Ability to adapt to department growth, expansion, and policies/ procedures, required.Experience in pediatrics required.Basic knowledge of Microsoft Office Programs, such as Excel, required.Proficient knowledge of electronic health records, required.Ability to offer flexible work hours, including the ability to work weekend hours, required.
