LHH is assisting a local nonprofit in hiring a Case Manager! Our client located on the northside is seeking a dedicated Case Manager to assist their community members in areas of crisis counseling, education, and connection to resources. The ideal candidate for this role will have 1 year of Case Management experience and is open to a flexible schedule.Summary:Hire type: Contract to Hire
Location: 100% onsite, near Edgewater neighborhood
Hours: Monday – Friday, standard business hours; some evening and weekends (rotation)
Responsibilities:Assist clients through crisis management, needs and service planning, and case management/advocacy
Provide education and information on resources to clients including employment and health resources
Assess housing needs, assist clients in securing housing, and planning logistics
Update and maintain client files, reports, and ensure proper guidance and support throughout the program
Qualified candidates possess:At least 2 years of experience in case management, transitional housing, or crisis counseling
Higher education preferred
Must be compassionate and a strong advocate for our clients
Must be passionate about social services
Must be available for weekend and evening rotation
