As a Field RN, you will play a crucial role in an Integrated Care Model by sharing essential information about member needs and advocating effectively. If you reside in New York City, NY, you will have the flexibility to work remotely while addressing significant challenges. Expect to travel within Queens, NY. Primary Responsibilities: Assess, plan, and implement individualized care strategies for patients directed toward the most suitable and least restrictive level of care. Utilize the NYS UAS Assessment as needed in the home or via telehealth to create a person-centered service plan to meet member needs. Identify and refer patients to relevant social service programs, including financial, psychosocial, community, and state supportive services. Manage a continuous care plan as the primary point of contact throughout the care continuum. Communicate necessary health-related information with all stakeholders to ensure prompt and quality care and services for all members. Required Qualifications: Current, unrestricted, independent licensure as a Registered Nurse. 2+ years of relevant clinical work experience. 1+ years of community case management experience coordinating care for individuals with complex needs. Demonstrated knowledge of home health, hospice, public health, or assisted living. Proficiency with MS Word, Excel, and Outlook. Willingness and ability to travel within the assigned region to visit Medicaid members at home or in other settings such as community centers, hospitals, or provider offices. New York state-issued ID or the ability to obtain one prior to hiring. Reside in New York City, NY. Preferred Qualifications: Degree in Behavioral health or clinical field. Experience with electronic charting. Experience securing community resources. Field-based work experience. Background in managing populations with complex medical or behavioral needs. The hourly range for this role is $28.03 to $54.95 per hour. Pay is determined by various factors, including local labor markets, education, work experience, and certifications.
