Senior Case Manager (Oakland)

LHH Recruitment Solutions is seeking a dedicated Case Manager for our healthcare client in the Oakland area. This is a contract-to-hire role involving both fieldwork and in-office administrative tasks. The position focuses on assisting patients with their care plans and healthcare needs, as well as providing support in areas such as housing, behavioral health, and documentation.Key Responsibilities:Assist patients with developing and managing care plans.Provide support for additional services including housing, behavioral health, and documentation.Work with clients experiencing complex health issues, including homelessness, mental health challenges, and substance abuse.Navigate and utilize community resources effectively to support clientele.Requirements:High School Diploma or equivalent required; Bachelor's Degree in Psychology or Social Services strongly preferred.Minimum of 2 years of case management experience.Proficiency with Microsoft Office and comfort with learning new software systems.Valid Driver’s License required, as this role includes fieldwork.Skills:Compassion and empathy for helping others.Excellent written and verbal communication skills.Ability to thrive in high-pressure environments and adapt quickly in fast-paced situations.Understanding of community resources relevant to client needs.Ability to de-escalate situations calmly and effectively.Strong interpersonal skills to connect with a diverse range of individuals.If you are passionate about making a difference and have the skills and experience we’re looking for, we encourage you to apply!Pay Details:
