Our EFFC program serves 137 youth ages 0-21+. It supports them with a multidisciplinary team that includes a Case Planner, Health Services Team, Psychiatrist, Behavioral Support Specialist, Family Advocate, Case Aide, Family Team Conferencing Coordinator, Education Workforce Specialist, Employment/Internship and Housing Specialist, Fair Futures Coach, Foster Home Recruiter and Licensor, and Clinician. Our therapeutic approach to foster care includes direct access to Clinicians and foster parents who are trained at a treatment foster care level implementing the ARC framework. This makes a difference because our team approach provides a space for healing while providing skill-building and future-focused planning for youth in our care.Cayuga Centers is looking to hire a passionate and dedicated Family Team Conference (FTC) Facilitator for the NYC Enhanced Family Foster Care (EFFC) program to engage families, community members, and service providers in critical decisions related to safety and risk. The FTC Facilitator will provide assessment, engagement, case management, and other support services, which help ensure that children have strong families to grow up in and have the preparation and opportunity to succeed in school and life. Our FTC Facilitator also ensures that responsive action is taken to address families' needs, accurate and prompt communication, and delivery of services in a respectful and culturally appropriate manner.How you get to use your skills…Facilitate family team conferences, including six-month permanency conferences, placement preservation conferences, and goal change conferences, many of which occur in-person.Collaborate with ACS facilitators when blended conferences are required to provide third-party review of conferences facilitated by ACS.Maintain fidelity to the FTC model. Establish an atmosphere conducive to facilitating an FTC that is strength-based, solution-focused, and engages all stakeholders in a participatory process to assist in reaching consensus.Ensure all safety and risk concerns are identified and addressed in conferences.Complete administrative responsibilities, including the timely documentation and data entry of family team conferences in CONNECTIONS.Facilitate a minimum of two to three conferences per day, many of which occur in-person.Attend inter/intra agency meetings to share information, develop policies and procedures.Ensure that case planners/therapists have invited all the necessary participants and that families have been encouraged to include community partners who will support them.Collaborate with the treatment team & CQI to ensure compliance with all conferencing mandates and policies by the set deadline.Conduct pre and post FTC briefing of case with the treatment team to note coaching and training needs.Participate in weekly supervision, program meetings, trainings, and workshops to enhance professional development and advance program development initiatives.Schedule: Non-ExemptRegular hours entail 9:00am - 5:00pm, must be available in the evenings - Monday - Friday. Must be flexible.Experience and Skills:
