What Youll Be Doing:

• Implement and monitor individualized patient care plans, adjusting as necessary to meet specific patient needs.

• Serve as the primary care coordinator, collaborating across disciplines to deliver well-integrated care.

• Monitor, record, and effectively communicate patient conditions, ensuring that all physician and nursing orders are accurately followed.

• Play a key role in planning patient discharges, working alongside the healthcare team to support patients recovery and transition.

Requirements:

• 2+ years of recent experience in a medical-surgical environment.

• Proficiency with Cerner electronic health records system.

• Ability to float to other medical-surgical areas as needed.

• Specialized orthopedic experience preferred.
