The Home Care Services Department comprises the Home Health, Hospice, and Palliative Care Programs. Home care nurses work collaboratively in an interprofessional team environment to provide the highest level of patient-centered care focused on supporting patient goals. These programs offer patients the opportunity to receive skilled nursing care in the comfort of their home.

Responsibilities
The Wound Specialist Home Care nurse is responsible for care of patients with complex wounds requiring consultation in the home. Additionally, the Wound Specialist Home Care nurse provides education to home care staff and other collaborative home health agencies. This position will require the wound nurse to perform occasional audits, support performance improvement efforts, and case manage a select group of patients in the home setting. The Wound Specialist will support efforts to increase wound care collaboration across the continuum of care and will interface with inpatient and clinic wound specialists to ensure patients receive seamless care.
Required Qualifications
At least 1 year of recent RN Experience as a WOCN or CWON in an acute care or home health setting
WOCN or CWON certification
Graduation from an accredited school of nursing
Possession of a current California RN license
A Valid California driver's license
Advanced wound care including sharps debridement
Demonstrates accountability and professionalism
American Heart Association (AHA) BLS CPR Certification
Preferred Qualifications
At least 1 year of Home Health Experience in a Medicare-certified home health agency
At least 2 years of inpatient experience as a bedside RN on a nursing unit

Special Requirements
Employees must participate in the DMV Pull notice system
Has mandatory reporting responsibilities for domestic violence, child abuse, elder/dependent abuse
Must demonstrate ability in assessment, planning, implementing, and evaluating patient care, taking into consideration protective interventions, development (behavioral and physical) and physiological characteristics specific to the patients
Ability to safely lift 25 lbs. and assist in moving patients and stand on feet over extended periods.
Must be able to travel to patient homes
Some restricted vacation due to program needs is possible
Must work evening, on-call, weekends, holidays, and overtime as defined by the collective bargaining agreements, will act as relief night charge nurse on occasion
References reflect an overall meets or exceeds
This position may be subject to a criminal background investigation, drug screen, Live Scan fingerprinting, medical evaluation clearance, and functional capacity assessment
