Contemporary apartment designs and upscale amenities enhance our commitment to fostering health, camaraderie, and lifelong learning opportunities.Position OverviewWe are seeking an On-Call APU Social Worker to provide support and assistance to patients in need at our healthcare facility. The Social Worker will collaborate with interdisciplinary teams to assess patients' needs, develop care plans, and connect them with appropriate resources and services.ResponsibilitiesConduct assessments to identify patients' social, emotional, and financial needs.Develop and implement care plans in collaboration with healthcare team members.Provide counseling and support to patients and their families.Connect patients with community resources and services.Maintain accurate and timely documentation of patient interactions.Participate in interdisciplinary meetings to discuss patient care and outcomes.QualificationsBachelor's or Master's degree in Social Work.State licensure as a Social Worker.Experience in a healthcare setting preferred.Strong communication and interpersonal skills.Ability to work effectively in a fast-paced environment.
