Contemporary apartment designs and upscale amenities enhance our commitment to fostering health, camaraderie, and lifelong learning opportunities.Position OverviewWe are seeking an On-Call APU Social Worker to provide support and assistance to patients in need at our healthcare facility. The Social Worker will collaborate with interdisciplinary teams to assess patients' needs, develop care plans, and connect them with appropriate resources and services.

ResponsibilitiesConduct assessments to identify patients' social, emotional, and financial needsDevelop and implement care plans in collaboration with healthcare team membersProvide counseling and support to patients and their familiesConnect patients with community resources and servicesMaintain accurate and timely documentation of patient interactionsParticipate in interdisciplinary meetings to discuss patient care and outcomesQualificationsBachelor's or Master's degree in Social WorkState licensure as a Social WorkerExperience in a healthcare setting preferredStrong communication and interpersonal skillsAbility to work effectively in a fast-paced environment
